Kendal Crosslands is seeking a Full-time Nurse Practitioner - 5 days a week with an occasional Saturday, 4-hour shift. No holidays!

Nurse Practitioner
Responsibilities:
- Working with Kendal~Crosslands Communities primary physicians within a Collaborative Practice Agreement to provide medical on-site care and services for residents in our community.
- Identifying and managing chronic and acute health conditions by requesting laboratory and/or x-ray studies, initiating treatment, and/or managing medication regimens with appropriate follow-up.
- Responding to emergencies and administering first aid to residents by evaluating injuries and arranging further treatment if necessary.
- Participating in planning for residents’ needs as they transition through levels of care.
- Collaborating with the Director of Resident Care on any present or anticipated problems.
- Performing medical procedures as necessary.
- Adhering to Pharmacy protocols and procedures, including e-prescribing.
- Administering treatments, injections, and medications to residents as prescribed/necessary.
- Communicating lab and test results to residents and physicians.
- Managing INR Lab results and follow up as necessary.
- Providing wellness screenings.
- Initiating requests for consultations or referrals.
- Participating in Community Quality Assurance, Pharmacy, and Infection Control Committees as requested by the Director.
- Arranging and establishing orders for transfer to the Health Center or hospital and assisting with discharges from the hospital back to Independent Living and/or to Skilled Nursing.
