RNAC/Registered Nurse Assessment Coordinator

We are seeking a passionate Registered Nurse Assessment Coordinator to join our team. Barclay Friends is a Quaker values-based Skilled Nursing, Personal Care, Memory Care, and Post-Acute Rehabilitation center.
Essential Functions:
- Ensures the MDS process runs effectively: Completes care plans with the Interdisciplinary team on a timely basis. Performs assessments of all residents to generate the MDS and monitors the accuracy of MDS and Nurse assessments prior to transmission. Completes the submission of the electronic record within 30 days of MDS completion. Ensures that assessments are completed and signed in a timely fashion and discusses any concerns regarding compliance of team members with the DON or Administrator.
- Manages the MDS process for the skilled nursing unit of 64 beds: Maintains a working knowledge of MDS requirements and RAI process. Monitors interdisciplinary team members to ensure that documentation is timely and complete. Maintains current resident assessment schedule and identifies staff responsible for completion of assigned sections of assessment.
- Maintains the RAI manual with current updates and completes monthly reports with analysis on schedule.
- Coordinates the Restorative Nursing Program by working with Therapy in communicating the resident’s restorative program to the Certified Nursing Assistants (CNA), making sure the Care Plan is updated to reflect the restorative goals, auditing the CNA documentation weekly for compliance and notifying the DON of non-compliance.
- Leads various Quality Improvement committees and workgroups, such as the MDS/Documentation Team and Infection Control Team. Attends and participates in quarterly QAPI meetings, communicating thoughts and directives clearly and effectively to the team. Participates in team problem-solving. Practices flexibility with scheduled working hours as needed to meet department needs. Conducts and attends meetings as designated.
- Has the option to work from home 1 day a week.
Qualifications:
Graduate of an approved school of Registered Professional Nursing and current licensure by the Commonwealth of Pennsylvania as a Registered Professional Nurse. Must maintain an active CPR Certification. Minimum of two years’ experience in a Long Term Care Setting as an RNAC. Excellent computer skills and strong clinical assessment and leadership skills are required. Effective organizational, problem-solving, writing, and interpersonal skills are required. Must be detail-oriented.
